1. All adults (over 18) and minor guests (under age 18) must complete page 1 & 2.  Parents or Guardian’s complete page 3 (Indemnity page).
2. Be sure to complete all name, address, signature and date fields.
3. Attach legible ID to lower left hand corner of page 1.  ID’s can be attached as separate page.  Any other applicable Proof-of-Discount can be attached as a separate page. For minors attach a birth certificate, school ID or passport that shows DOB.
4. Scan only signed pages of completed waiver.  Attach to email as acceptable document such as PDF, JPG, JPEG, BMP. Email  chris@uguidesdpheasants.com

Other Important Notes

• We require that all documents be scanned and emailed. In event you are not able to scan and email your documents please make other arrangements such as bringing your forms to FedEx, Kinko’s or mail them to someone in your group that has that print/scan/email capability.
• Faxing or Mailing are NOT acceptable options in our hunting system.
• If you have any questions please contact your Group Coordinator.

When full payment and registration is complete for a group, UGUIDE will email the Group Coordinator a Destination Pack containing: Landowner contact info and directions to your destination, pheasant camp amenities, area amenities, maps of property (in some cases landowner provides these on arrival), packing tips, info on how to obtain your license, any policies specific to your location.
